What all exactly is needed to convert my 454 crew cab 3/4 ton Chevy truck to 6.5L diesel?

Not very much, really - you'll need an engine, diesel flywheel (for manual trans) or flexplate & torque converter, diesel or EFI fuel sender (you need a return line), diesel radiator, and a pushbutton switch and generic starter relay to fire the glow plugs. If you have a TH400 you'll probably want to get a different modulator, and you'll also need the Vacuum Regulator Valve (VRV) parts for the injector pump.
