Dual mass flywheel

Does anybody know what the part# of the Duramax dual mass flywheel is. Maybe someone has an warranty ticket with it on there still.

I had ny flywheel replaced under warranty due to flywheel failure.

Flywheel, 0.666, part #: 12569363
Plate, 0.859, part #: 12569364

Cylinder, 0.775 part # 12572805
Cylinder, 0.780, Part # 15019756

The "0.XXX" are group numbers I believe.

The cylinders are clutch master cylinder and throw-out bearing. The bill does not explain which cylinder is which part.

The dealer replaced the hydraulic parts because they failed at a stop light. The flywheel had been acting up for some time, but I had to go thru the computer "reflash" routine before the dealer/GM belived I had a flywheel problem.
