DTC P0406

I am setting a P0406 code (EGR flow sensor/high input). I am a novice to the diesel world and am really confused.
Is this due to low intake air flow?
Where does the ECM pick up this reading from?
The code was set during normal in city driving.
This is on '96 Chevy 6.5TD (S series vin code)in a 1500/Z-71.
Any help is appreciated...

You might want to post your question on the 6.5L Turbo Diesel forum. Dr. Lee knows all, but he doesn't own a truck like yours - his is a 1984 C10 with probably the nicest 6.2 ever built.
